I'm writing this, because I'm getting dizzy, I don't know how to use proper English anymore. Please help!

What should the heading on top of a page say?

Think of any newspaper headline and it would go something like:

Boy killed blaze.

They'd then go on to say in opening paragraph:

A boy was killed today...

The indefinate article (a) is dropped - usually to catch attention. Your examples are no different. You can have it with the definte article (the), or without it. It just depends what effect and formality you're after. The thing with titles is that they mess around with normal structure of a sentence so you wouldn't be shot for using the second here.
